Kindly download and complete our online application form (below) and prepare a 2x2 photo of the child.

Once your child's application form has been submitted, we will review and schedule an interview for you and your child.
Enrollment
We are so happy to have you on board The Discovery Academy of Innovation!
After assesment and interview, kindly download and complete our fillable Enrollment form (below) and Health Assessment form.
Submit along with the other required documents listed in the Enrollment form.
